How We Approach Joint Replacement

Joint Replacement

Joint replacement is the last step, not the first.
Dr. Wallentine and Dr. Hoopes begin with the whole picture: your imaging, your activity, your goals, and what you have already tried. When replacement is right, Dr. Wallentine plans it on the Mako robotic platform for knees and hips. The robot does not operate. It holds the plan to the millimeter so that the implant sits exactly where the planning scan said it should. Most of our hip replacements use the direct anterior approach, which works between muscles rather than cutting through them. Many patients walk the same day.

What We Treat

Osteoarthritis of the hip and knee, inflammatory and post-traumatic arthritis, avascular necrosis of the hip, hip fractures in older adults, and hip or knee replacements that have loosened, worn or become infected. We also run a conservative-first lane for knee pain in adults over fifty, where the goal is a plan before anyone talks about surgery. If a joint has already been replaced elsewhere and hurts, we evaluate it the same way: find the cause first, then decide.

All 5 joint replacement procedures
Hip and knee replacement planned on the Mako robotic platform, plus revision surgery when an earlier joint has failed.
All 6 knee conditions
Knees tell us a lot by how they hurt: a pop and swelling, a catch, or stiffness that built over years.
All 5 hip conditions
Hip problems show up in the groin and thigh, not the side, and they fall into two very different groups.

When to See Us

When a hip or knee limits walking, stairs or sleep despite anti-inflammatories, activity changes and an injection, it is time for a conversation about what comes next, even if the answer is not surgery yet.A fall with hip pain in an older adult is seen the same day. A replaced joint with new pain, swelling, drainage or fever is seen the same day.A second opinion before a replacement is welcome and common.

Conservative and Older-Adult Knee Care

Not every painful knee over fifty needs a new one.Dr. Hoopes built a lane for exactly this patient: knee pain in older adults that deserves a real plan before anyone talks about replacement. That plan can include bracing, targeted strengthening, weight-bearing adjustments, injections and careful follow-up. If the knee eventually needs replacement, Dr. Wallentine is down the hall and you keep your record. If your knee hurts and you are over fifty, start with Dr. Hoopes or Dr. Wallentine. Either way you will leave with a diagnosis and a plan, not just a referral.

Your First Visit

Standing X-rays of the joint, taken on site, show how much cartilage is left and how the leg lines up.The exam checks motion, stability and the joints above and below.Dr. Wallentine reviews the images with you on the screen. If replacement is the right step, a CT scan follows so the Mako robotic plan can be built before surgery. You will know the options, the likely timeline and what to do in the meantime before you leave.

What Does Robotic-Assisted Mean?

Before surgery, a CT scan builds a three-dimensional model of your knee or hip.Dr. Wallentine plans the implant size and position on that model, then the Mako arm keeps the saw or reamer inside the planned boundary during surgery. The result is a cut that matches the plan rather than the surgeon-of-the-day estimate. Studies associate robotic assistance with less soft-tissue disruption and more consistent alignment. It is still your surgeon doing the operation.
Dr. Wallentine is registered on the Stryker Mako surgeon finder for total knee, partial knee and hip, after more than three decades of joint reconstruction.
