What it is

Avascular Necrosis of the Hip

The ball of the hip has a fragile blood supply.
When it is interrupted, the bone inside dies, weakens, and eventually collapses, taking the cartilage with it.It strikes younger adults than arthritis, often between thirty and fifty.

Both hips are affected in about half of patients.

Causes

Long-term or high-dose corticosteroid use, heavy alcohol use, hip fracture or dislocation, sickle cell disease, and some blood and autoimmune conditions. Often no cause is found.A history of steroid treatment is the most common thread.

How We Diagnose It

Early disease is invisible on X-ray, so MRI is the key test and is ordered whenever a younger adult has unexplained hip pain. X-rays stage the collapse once it has started.Staging decides whether the joint can be saved.MRI is ordered at the first visit for younger adults with unexplained hip pain; early disease can sometimes be treated without replacement. Dr. Wallentine and Dr. Christensen operate at Utah Valley Hospital and Mountain View Hospital.

Treatment Options

Early, pre-collapse disease may be treated with core decompression, drilling into the dead bone to restore blood flow, sometimes with bone graft or biologics, plus protected weight bearing.Once the ball has collapsed, total hip replacement is the reliable answer, and the direct anterior approach suits these younger patients well.
