What it is

Reverse Total Shoulder Replacement

A reverse replacement places the ball on the socket side and the cup on the arm side.
That changes the mechanics so the large deltoid muscle can raise the arm without a working rotator cuff.It was designed for shoulders where the cuff is torn beyond repair and the joint has worn as a result, a condition called cuff tear arthropathy.

Who It Helps

Candidates have a massive irreparable rotator cuff tear, arthritis with cuff failure, certain complex shoulder fractures in older adults, or a failed previous shoulder replacement. Most are over sixty-five and have lost the ability to lift the arm overhead.

What to Expect

Surgery takes about two hours under general anesthesia with a nerve block, with a one-night stay for many patients.Dr. Taylor plans the implant position from a CT scan and discusses expected motion honestly: a reverse restores overhead reach and relieves pain, but rotation behind the back is often limited. Many reverse replacements here follow a fracture from a fall on ice; the goal is a shoulder that reaches a cupboard again by spring. Dr. Taylor operates at Utah Valley Hospital and Timpanogos Regional Hospital.
Reverse Total Shoulder Replacement

Recovery Timeline

A sling for three to four weeks, with gentle motion beginning early. Most patients lift the arm overhead within six to eight weeks and reach the bulk of their recovery by four to six months. We recommend avoiding lifting more than about twenty-five pounds with the replaced arm for life.
Milestones
Week 1Gentle motion begins
Week 3 to 4Sling off
Week 6 to 8Arm overhead
Month 4 to 6Full recovery

Risks and Considerations

Instability or dislocation of the new joint is the main early risk.Notching of the shoulder blade bone below the implant, fracture of the acromion, and infection are others.Long-term implant survival is now well documented and improving with modern designs.
